What each is for, where to get it, and how often colleagues are asked to complete it.

Notes on completion
Forms are held by the office named and are not kept on the floor. Colleagues who need one should ask; there is no procedure for asking and no record is made of having asked.
Form 12 is completed on arrival and again on return. Colleagues occasionally remark that it feels familiar. It is a short form and takes a few minutes.
The Undertaking has no number because it predates the numbering. It is not issued, is completed once, and colleagues do not retain a copy. Personnel are not able to say more about it than that, and have asked us to record that they have been asked.
